Submitting a Request

The Fire Department of the City of New York commonly provides the public with access to records in compliance with Federal, State and NYC laws. Below are instructions for obtaining commonly requested records by Mail, In Person or Online through Open Records, along with forms that are needed to complete the process. Note all mail requests must include a check or money order for applicable fees payable to 'NYC Fire Department' (no cash accepted) and a stamped, self-addressed envelope. Subpoenaing Records

Except as may be authorized by federal or New York State law, all subpoenas for Fire Department records must be 'so ordered' by a judge. The so-ordered subpoena, together with the applicable subpoena fee, may be served on the Fire Department in-person in accordance with the requirements of the New York Civil Practice Law and Rules, to:

Request Report

Incident reports can be requested using a mail-in form. All forms must be mailed with a $10 search fee (payable by certified check or money order) and a self-addressed stamped envelope to return the response. Anyone can request a crash report as long as the required information can be provided. If a vehicle crash occurred in Massachusetts and has been filed and processed by the Registry of Motor Vehicles (RMV), you can request either a standard or a certified copy of the police crash report. How to request a refund Go to reportaproblem.apple.com. Sign in with your Apple ID and password. If you see the 'Report' or 'Report a Problem' button next to the item that you want to request a refund for, click it. To submit a request using the Consumer Request Form, you are required to provide your First Name, Last Name, Street Address, City, Zip, and Date of Birth. You are also required to provide one of the two following optional fields: Social Security Number or Your Driver’s License Number and State.

You will be advised of the exact amount due for reproduction costs, if applicable.

FOIL Requests

All Freedom of Information Law (FOIL) requests to the New York City Fire Department should be submitted via NYC OpenRecords. Select Fire Department of New York City (FDNY) from the agency dropdown menu.

Request and Receive a Report Electronically

You can request and receive reports from LexisNexis Risk Solutions electronically. After your request is submitted, you will receive a letter via U.S. Mail with details explaining how to access your report electronically.

If you prefer to mail in your requests, you can choose to download a printable PDF form send it to us via U.S. Mail. If it is more convenient to call us, feel free to give us a call and we will submit your requests for you.

If you received an Adverse Action Letter

An adverse action letter is sent when a carrier has not given a consumer the best rate for insurance or taken any of the following actions:

  • Denied you insurance
  • Increased your rates
  • Limited your coverage
  • Cancelled your policy

If your insurance company has sent you an adverse action letter, please contact the LexisNexis Consumer Center at 1-800-456-6004 to request the information related to the adverse action. Please be ready to provide your first and last name, Social Security Number, driver’s license number and state in which it was issued, date of birth, current home address and phone number. If your insurance carrier provided a reference number on the adverse action letter you received, please be ready to provide it, as well. This will help expedite the process.
